Full job description

Milestone health solutions is looking for an individual to provide adult mental health case management in the community and/or remotely to adults that are diagnosis with severe mental illness and dual diagnosisThis is a contractor position; up to 10hrs week.

Education requirement:

At least a bachelor degree in the human service field.
Must be a qmhp-a.

Experience:

At least one year of face to face clinical work experience with adults that have a diagnosis of severe mental illness.

Individuals will have knowledge in the following areas:

Experience with assessments and functional assessments
Developing individual service plans
Coordinating and linkage to services in the community
Experience with discharge planning
Ability to work independently
Good communication and writing skills
